This form is for programs to develop a Quality Improvement Plan after receiving a playground safety inspection report from a Certified Playground Safety Inspector (CPSI). The intent is to itemize and prioritize hazards that have been identified and to begin to create a schedule and/or plan to mitigate those hazards.
